Chicken Pox is caused by the varicella-zoster virus, a DNA virus from the herpesvirus family. It is primarily transmitted through respiratory droplets or direct contact with vesicular fluid from an infected person. After initial infection, the virus remains latent in the dorsal root ganglia and can reactivate later in life as shingles (herpes zoster).

Risk Factors

  • Age: Common in children under 12 years.

  • Immunocompromised status: Individuals with weakened immune systems are at increased risk of severe disease.

  • Lack of vaccination: Unvaccinated individuals are at higher risk.

  • Close contact with infected individuals: Family members or schoolchildren in proximity to infected persons.

  • Pregnancy: Can lead to complications for both mother and fetus.

Pathology

After initial infection, the virus replicates in the nasopharynx and regional lymph nodes, followed by viremia (presence of virus in the blood), leading to dissemination to the skin. The virus causes necrosis and inflammation in the epidermis, forming the characteristic vesicles. 

The lesions go through stages:

 macule → papule → vesicle → pustule → crust.

In adults and immunocompromised individuals, VZV can cause severe complications like pneumonia, encephalitis, and disseminated infection.

Clinical Features

  • Incubation period: 10–21 days (average 14 days).

  • Prodromal symptoms: Mild fever, malaise, anorexia, and headache (more prominent in adults).

  • Rash: The hallmark symptom. It starts on the trunk and face, then spreads outward. Lesions appear in crops and are intensely itchy.

  • Fever: Often precedes or accompanies rash onset.

  • Other symptoms: Sore throat, irritability in children.

Diagnosis

  • Tzanck smear: Shows multinucleated giant cells.
